What are you running the course from? An LMS? The authoring tool itself?

From an LMS, I also tested it on SCORM cloud and the same issue is experienced.

Ah sorry I thought it was failing to connect to the LMS but I wasn't reading the output carefully enough. Could you just confirm what version of the framework and the assessment extension you're running?

Hmm that is a bit of an old version - 2.0.9 is current and we'll be releasing 2.0.10 fairly soon.
Could be worth installing 2.0.9?
You could always export your course source and test the upgrade first by doing it in the exported version
