What can cause the problem?

Lots of things, potentially! That's really too open-ended a question to answer.

What I can say is: if I download a fresh copy of the framework, remove all questions (and their containing blocks) bar the first from the assessment, set the remaining question to 'full width', switching off banking and set the passmark to 100 in the assessment config, it works perfectly for me.
